I encountered this problem on GOG version.

I setup BMOUSE as external controller. I setup controller combination as Keyboard and External. Every time I run Blood it does not work unless I go directly through setup. I mean I have to run SETUP.EXE and "Save and launch Blood" and only than the mouse works. It is like it does not save the settings or something like that.

Of course I click mouse aiming hotkey if somebody would wondered if it is the problem.

I wonder if it isn't saving correctly, two things to check in your Blood.cfg file, at line 127 and line 130:
ControllerType = 3
ExternalFilename = "BMOUSE.EXE"

Failing that, you can edit your dosboxBlood_single.conf to launch with BMOUSE.EXE directly
BMOUSE.EXE LAUNCH BLOOD.EXE

Thank you very much for your help. My Blood.cfg was setup correctly so I tried to launch the Blood through BMOUSE as you suggested, and now it finally works without a problem.
