Win 10, no mods. Seems to run fine except there are no sounds effects. Can't hear gun fire or doors, just music playing.

I don't have the game, but I believe it uses Dosbox.

I did a quick search and it seems you need to do one of two things:
1. Edit the Dosbox configuration file and change the default IRQ to match what you set in the game setup.
or
2. Run the game setup and change the IRQ to something else (probably IRQ 5 or IRQ 7, depending on what Dosbox is set to).

Blood 2 is a win32/DirectX game. Blood 1 uses DOSBox.

You are correct.

In that case, I'd set the executable to compatibility mode for Windows XP and then in the pre-game options menu I'd try different sound settings.
No luck. :(
I don't have a dosbox config file. I tried selecting no 3d sound as well as the other 3d sound drivers. Also not seing any kind of game setup file either.

I had this problem after messing with the 3d Sound settings in the launcher. Apparently even if you set it back to No 3D Sound, the autoexec file keeps your previous setting. So I'd say just delete Autoexec from the game directory and restore the old version from the installer or GOG Galaxy Verify option.
Apparently this is not compatible with Win10. I emailed support and other than suggesting another computer without Win10 they couldn't do anything.

I had the same issue under Windows 10 and the solution for me was hilariously simple...

Open the autoexec.ini
Find the entries for "max3dvoices" and "maxswvoices" and make sure it's not set to 0.
Set them to 32, 64 or higher if your sound card supports it.

Cause if it is set to 0, no wonder no sounds are played at all xD

This is a great find - using setup 3D sound settings always resets "max3dvoices" and "maxswvoices" settings in autoexec.ini to 0, so after making any changes, quit setup, fix entries in autoexec.ini and then start the game without any more attempts to change the 3D sound settings. I can confirm EAX working with ALchemy on supported hardware in this game on Windows 10.
