In the game launch window, have you tried disabling DirectX 6 and joysticks options? They are found in the "Advanced" section.

Yes! I have and it's still not working.

Can you be a little more specific about what you have tried already?

BUMP! Hope someone can help me with this.

I have tried:

Reinstalling
Going into Autoexec and Turning Mouselook from 0 to 1
Downloading Many Patches
Disabling Joysticks
Disabling DX6 Commands

I HAVE TRIED LOTS BUT TO NO AVAIL.

I want the game to work. I found out something though. That look up and down is binded to Up and Down arrows. If that is the problem, how do I rebind them to the mouse?

What kind of mouse do you have? Is it a plain-jane USB, bar of soap, or is it a souped-up 28 button Razer monstrosity?

Oh. My old mouse died on me so I have a new mouse. It's a Platinum Wireless Laser Mouse. Did not help the problem at all. The mouse has:

Left And Right Buttons and Scroll Wheel.
A windows button
A Forward and Backward button for searching the web.
A DPI Button

So I have no clue what to do now.

Just trying to elevate variables. The thinking being that if you have something fancy that requires extra drivers, there might be some translation issues between Windows 10 and Blood 2. But if you had something simple before the new laser mouse and it didn't work then, it might not be that.

I didn't have Blood 2 setup with the most recent install of Windows 10, so I though I'd document what I did. In the end things were working fine.

As far as hardware on this workstation I just have:
a plain 2 button + scroll wheel/button, USB wired, optical Dell mouse
a nVidia GeForce GT 610

See if I did anything different from what you had done.

Blood 2 Configuration Steps
Installed Blood2 to C:\games\Blood2
Downloaded the dgVoodoo2 packages and unzipped:
D3D8.dll
D3DCompiler_43.dll
D3DCompiler_47.dll
D3DImm.dll
DDraw.dll
DE_Msg.dll
dgVoodooSetup.exe
into the Blood2 install directory.

Launced Blood2.exe
Picked the Display button and selected "dgVoodoo DirectX wrapper(display)" also set my Resolution to my monitor's max ability "1920 x 1080 x 16"
Picked the Advanced button to disable directx 6 commands and joystick.

Attempt 1 to launch Blood 2

Was prompted to install DirectPlay.
Installed DriectPlay, was prompted that the client might have to restart.
Client MFC Application has stopped working.

Launch Attempt 2
In game: Had to set the display settings
In game: Options>Mouse: Ensured Mounslook was "On"
Client MFC Application has stopped working.

Restarted Windows

Launch Attempt 3
In game: Display settings did not save, so I reconfigured it. Again I picked "dgVoodoo DirectX wrapper(display)" but I also selected a lower resolution: 1280 x 800 x 16
Blood 2 seems to run fine now. I was able to play through the first subway level. No problem with mouse look.

Hey dude, I just reinstalled it and installed the latest version of DgVoodoo 2 and guess what? ITS WORKING! Mouselook is working fine again and I don't know what the hell happened! I guess a fresh install did help this time and turning the resolution down was a good idea! Now I have to figure out how to stop lag! Thanks!

Sounds like the computer djinn were appeased at some level, Not sure where the lag is coming from. Just for grins and giggles, have you tried the software render, is there lag there?

I don't know if you have figured it out already, but the game glitches out if you have high framerates. If dgVoodoo2 forced vsync then it caps the framerate at 60fps (or whatever refresh rate you have) so probably this is why it works now. For future reference (and in case you want to play with vsync off which IMO improves the responsiveness) you'll need to use a frame capping program like RivaTuner Statistics Server to force the game run at maximum 60fps, otherwise you'll have problems again.

This applies to other LithTech games, like Shogo, NOLF, etc and even FEAR and Condemned (in those last games you wont have mouse issues, but there are other glitches like broken effects and -especially in Condemned- getting stuck in small objects for a few frames).

Generally it is a good idea to play older games using frame capping because they were never tested in high framerates and often are broken.
Good to know! Thanks, badsector. Can't say I play any of the Lithtech games that much to have to worry about it, but I'll add it to the reference pile.
