I bought BG&E as I have very fond memories of the game and would like to play it again. But when I start it up the entire screen is turned 90 degrees anti-clockwise, so if I want to see the screen right I have to tip my head all over to the left.
The screen is resized down so it'll all fit inside the screen, leaving a large black strip down both the left and right side of the screen.
The game plays well enough, yet this bug makes it positively unplayable.
Is there some sort of setting I have missed?
Tried it in both 1280*1024 and 1024*768 resolution.

Thats really odd, do you have anything unusual in your setup? Like more then one monitor or one of those monitors that you can use either landscape or portrait?
No, nothing. A rather usual monitor (SyncMaster 930bf) at 1280*1024. No second monitor.
I've never tweaked with the settings to change it to portrait view either.
And this problem has not turned up in ANY other game...
What OS ? If it's Vista or Win7 try checking the 'Run as Admin' box on the shortcut.
Do the same thing for the setup tool before you try to change any settings with it.
I guess you could try windowed mode to see what happens. You can check out this post here for windowed mode

as a last resort you could try to rotate the screen on your monitor 90 degrees clockwise before playing the game. Most graphic card drivers have this option (right click on the desktop and go to graphics properties/options or click the tray icon of your video driver). Not really elegant, but hey, if it's the only way to make the game playable..
I found the problem eventually, and it's such a moronic thing it's weirn no one else have noticed it...

When selecting your resolution it seems that each setup is listed twice, but with the parameters switched. So, what I was looking for was 1280*1024, and I chose 1024*1280 instead and didn't notice. For some reason this turned the screen sideways. *facepalm*

That said, the game runs smootly and well... provided I set water detail to minimum. Otherwise the sound won't synch with the image because the graphics start to lag. Even when there's no water in the shot. Shoddy programming, but I'll survive... ;)
