Does this ever come in handy? I can't remember one instance in either game where a wizard casts polymorph.

Yeah, it's a pretty useless item overall. The only possible benefit I can see is around wild magic -- or multiplayer -- but that's like taking an umbrella into the desert 'in case it rains'.

Petrification effects also use the Polymorph save category; this is probably where it comes in most useful, as there's a fair number of these (in BG1 anyway, with all the basilisks; in BG2 it's quite a lot rarer, I can't think of anything but beholders and a few rare traps that can petrify you).

Basilisk gaze is save vs. polymorph? I could've sworn it was save vs breath weapon.

Edit: I typed petrify instead of polymorph. Sorry, it's getting late here.

It's neither. A basilisk's petrification gaze grants a save vs paralyze/poison/death at -4 penalty.
Give it to Neera along with the wolf cloak from the cave with the +1 con book
