so for instance i look at the accuracy of to hit something it says 25 i swear its more like 5 i get more critical misses then i do hits its kinda ridicules and this is with any monster not just hard ones

25% is pretty low.

Are you skilled in melee/bow/throwing/firearms?
Do you have an eye injury (check your journal)?
Is your background the one which gives you more critical misses?

Yeah in order to figure out whether this should be happening we need more info on your character. What are you stats and skill levels? If you're fighting melee, you'll want a decent dexterity stat as well as melee combat skill. Also, when you mouse over an enemy to attack it, and the 25% hit chance is displayed, are there other icons showing as well? These icons indicate certain factors that are affecting your chance to hit, such as low light conditions.

Your opponents may also have ranks in dodge. Dodge trumps Melee and can cause critical misses.
