The game has a complex story with long cut-scene sequences, but there are fewer and they are shorter compared to earlier Metal Gear Solid titles. Most of the gameplay mechanics introduced in Ground Zeroes are carried over: it is still an action game and stealth oriented but replaces the linear corridor design from most earlier titles with large open world environments that offer the player unrestricted freedom for the approach. The world has now a real-time day and night cycle and various weather effects that influence enemy behaviour, visibility and sound. The gameplay follows the recipe from Doom RPG as it is shown in the first person while being a turn-based role-playing game rather than a shooter and puts emphasis on the plot. Combat and movement are turn-based, allowing the player time to select their responses in combat. The player turns at 90 degree angles and moves space by space. One step or action by the player allows all other characters in the area to take one step or action themselves. The game takes advantage of its deliberately slow pace, encouraging players to take their time and check out every little corner, read the books on every bookshelf, and destroy all the furniture to see if anything is hidden within. Levels include underground passages and weapon development laboratories plus a level involving a moving vehicle.

Abandoning the post-apocalyptic themes of the original series, the story takes place in modern Japan. The player character is an unnamed college student who is attacked by demons along with his girlfriend; they are saved by the timely appearance of a suave demon-hunter named Kyouji. Shortly thereafter, Kyouji is found dead under suspicious circumstances. The protagonist is then trapped inside of a warehouse and murdered by a deranged killer named Sid. Rather than stay dead, the player finds himself in the reanimated body of the slain Kyouji, and escapes from the morgue.
