When I started the game I got audio but no video.
A black or pink screen until the cutscene is over.
I noticed there is a MCI to Directshow mapper included in WC4's folder. (dxmci.dll)
But the mpeg2.dll included with the game does not seem to call the mapper on my system.
Solution: Download mpeg2.dll from http://www.si-gamer.net/gulikoza/dxmci.html and replace the original file.
Note that the dxmci.dll version on that site is broken itself, so do not replace both files.
Note to GOG: You might want to look into this and add this solution to the support section.
