NeechB: Mouse Cursor Misalignment Issue Update.
1. General Tab / Appearance / Scaling Mode
a) Unspecified to Centered/Stretched, Keep Aspect Ratio - Mouse Cursor In-Game Aligns BUT
Looks correct / Game Window fits my monitor perfectly
1. I can move mouse cursor upwards off screen.
2. I cannot move mouse cursor downwards to the bottom of my screen - Stops about 1/3 of the monitor size away from the bottom as if there was a border there.
I have tried to force every resolution and my mouse cursor in game disappears even if I check/uncheck capture mouse.
I have tried every graphics card available. Only SVGA doesnt run the game.
Turned off Antialiasing.
That is as far I got. If anyone has better luck than me with this can you please post it here.

Apologies, but I wish I could help more. It may require someone who is more knowledgeable in dgVoodoo.
The only other thing I can think of is the fundamentally wrong .dll file in context to your system/monitor is installed. To test this, you can try the other .dll files (though apparently any 64 bit system should be able to run 32/86 bit program). Perhaps try the x64 .dll version of DirectX from the "MS" file and try that. Also, perhaps try the Glide .dll files from the 3Dfx file and try running it with Glide instead of DirectX. There is also the two readme's and the QuickGuide that can perhaps help with this as well.
Other totally random resolution/screen options you could try, though I have no idea if this will help, are:
in General tab: "Scaling mode"
"Keep window aspect ratio"
Maybe uncheck "Inherit Color Profile in screen mode" (what I believe fixed my problem)
Try windowed mode
Output API?
in DirectX tab: uncheck "Application controlled fullscreen/windowed state"
mess with the miscellanneous check boxes
I know that's pretty broad and random, but the one thing I've learned from running, emulating, and modding older games is that you never know what is causing/will fix the issue - as well as things that fix someone's issue may cause your issue. I also don't possess much technical knowledge of how this works, what software/hardware runs what games and such, so most of my success regarding game manipulation is brute force, trial/error, YouTube videos, and forums.
I wish I could help more. Hope it works out for you. Worst case scenario is a refund, especially with GOG's new 30 day refund policy, but this gives you some time to mess with the settings. Let us know if any of these help or not.
