QWEEDDYZ: Keep in mind - refreshrate is not related to FPS and "this game not support refreshrate" is wrong words. Well, it is right actually. But this is not related to what FPS supported. Refreshrate (Hz) is refreshrate. Response time is response time (ms). FPS is FPS (frames-per-second).
FPS ALWAYS affect game and physics speed.
There is reports (ofc mistakes always possible) that this game - TQ: Immortal Throne Gold - is work on 120 FPS. So probably force game to use 120 FPS should be ok. if some strange things still happening - force game to 60 FPS. You still got advantages of high refreshrate+low response time display (or HD TV). Display is display after all.
Vsync nowadays should be turned off permanently. Maybe some gsync-etc with choose how much fps should be. Video drivers allow to limit FPS without limiting refreshrtate (because it not the same things).
Or some 3rd party software. Sometimes (very rare) they required to force game to slowdown FPS.
But mostly turning off Vsync and use ingame limiters should be OK for most games. Sometimes video driver.
UPD: PCGW say something about game speed isuues on high framrate but not say anything conrete for original IT. Also there link to 120hz support (wrongly described as 120 supported too).
TQ Aniversasy Edition wiki article clearly say that FPS should be strickted to 60 to avoid glitches.
So it is. Game normal FPS is stable 60, not stable 120 or more. Refreshrate can be any else. It can cause some graphical glitches (horizontal lines screen desync) (not very noticable actually, it not like interlacing) or not. For perfect screen image refreshrate should be same as FPS (and you still get avantages even in that case, but not same as on high framerate) - and not FPS same as refreshrate.
Just chosoe what you prefer more - less "lacing" image (not so criticaly noticable) or higher refresharate. My opinion - second is better. If there is no problem in game - more refreshrate and more FPS is better. Maximum refreshrate, but still limited FPS - just differ numbers for each game.
Using vsync will increase FPS nowadays with high refreshrate displays. Should be turned off.
First of, without Vsync there will be screen tearing.
Secondly forcing 60 fps through video driver's control panel does work, but give a little washed motion to the game.
And finally. Titan Quest it goes like this: the better hardware you have, more FPS you will get in game. So without Vsync it can potentially run 300 fps, which will make game physics incredibly fast. Vsync will lock fps to whatever your monitor refresh rate is so let's say 75Hz. At 75Hz the game is still quiet manageable, but again it would be better if in-game framerate would be locked at 60 fps regardless of your PC's specks or monitor refresh rate.
Either way, I did write to TQH Nordic's tech. support about this. They said game devs will look into it, but looking how they still didn't fixed labyrinth's map I'm pretty much moved on from TQAE.
