I have over 600 hrs in the game and have finished all DLC. I had experienced some issues with texture tearing (Radovid's ship, cave interiors, Weavess mill in the swamp, etc.) but was able to do a "fix" that solved the problem. Recently while waiting for a different set of dev's to address game-breaking bugs with a DIFFERENT game, I decided to go back and do some of the scavenger hunts I had bypassed earlier. I noticed that Update 1.32 was available so I installed it (took 6 hours) AND, all of the texture tearing issues have returned!!! So, I reverted to version 1.31(A) which I had played for nearly 2 years with no issues, but the graphic issues now appear in the older version as well. I have looked at the verify/repair option but it states that it will re-install verison 1.32 as part of any repair process.

The game is still playable (to some extent) but the texture issues are very annoying and I would really like to get back to where I was 2 months ago - it begins to look like I cannot get there. My out-of-date graphics cards and slow processor worked like a charm under version 1.31(A) so they are not the problem so far as I can tell.

Assistance would be appreciated. Unfortunately I do not remember what the original "fix" was for the texture tearing issues so I am coming here to ask if anyone else has had the issue with update 1.32, or if they remember what they did to resolve the original graphic issues.
Graphics issues like you describe are virtually always because of corrupt installations, so verify/repair is the way to go. Also check you don't have any mods that are mucking things up. Patch 1.32 was only a Chinese localisation patch, so it won't be that.

I had a similar issue, with version 1.31, and it was fixed by doing what the repost below mentions. If this doesn't work for your GPU, you might want to post for me your PC specs, so I might suggest to you something relevant to your hardware and software.
Other than that, I strongly suggest contacting the PC support for issues requiring at least basic knowledge of how computers and programs work, since rarely will you find a related working solution in this forum.
Also always read the FAQ first, since they provide solutions to all basic issues, making it a fool's errand to repost/copy paste them on other threads.

-----Finally an answer from the PC Support has come in and it is the solution to this issue. I will quote the solution as it was sent to me via e-mail.

"Please go to your CCC (Catalyst Control Center) and reset your Tessellation to 'Use application settings'.
'Override' with 2x tessellation causes this issue. You can set it to 4x and above, but not 2x."

All qudos go to Ivan of the PC Support department.

And below is what solved my problem perfectly based on Ivan's solution.

" I would like to note that, initially my tessellation setting was set manually to "Off". The fix worked after manually setting the "Maximum Tessellation" to "x4",as you suggested, and starting the game. After that I restarted the game using the "Use application settings", and it worked like when it was set to "x4". Unfortunately that didn't fully solve the problem, as some "unnatural" texture edges were still displayed.
What fully fixed the issue was manually setting "Maximum Tessellation" to "x64" (the maximum level of tessellation) and starting the game. After that I restarted the game using "Optimized by AMD" and it worked perfectly, like when I had set "Maximum Tessellation" to "x64". I have no idea why it works like that, but it certainly did eliminate the issue."

Case closed.
