1. Use the verify/repair-function of Galaxy. (-> Witcher 3 -> More).
2. If it doesn't help: Upload your savegame and present the link here. Someone will try to help you eventually. (If you have installed HoS, then someone = me).
3. If you are using cheats, mods, scripts etc. your savegame may be corrupt.
