This has happened several times now, using both keyboard and controller inputs. Geralt puts his sword away during combat with no input from me. Uhhhh, why? And how do I turn off this 'background AI' behavior, because quite frankly, it's ruining the game.

Yeah, I notice this also. It seems that if enemy is far (but you are still in combat), Geralt puts sword away.
Yes, it seems to be based on a simple "no enemies within X radius" state - even if you're being actively chased/attacked by ones beyond that range.

I'd really like to be able to turn off this "helpful" feature.

I post about this in sugestion thread. Lets just hope it will make it to patch, because fighting againts archers is also hard with this feature.
In addition to that, it seems Geralt sometimes faces in weird directions when combating a locked on enemy. I don't know if this was intentional or not, but it feels very awkward fighting enemies without facing them

I think this is intentional. Target lock is just for camera. It would be hell when fighting with multiple enemies with lock on someone.
And what about deflecting the arrow?

When you're far enough from the enemy, Geralt put his sword away. Is the enemy still shooting? Or is Geralt covering himself?

Try manually drawing your sword before combat... I think he never puts it away then (until you do it manually).
I've not had him put it away early, but I have had him forget to draw it at all. Charging into a pack of wolves fists swinging was a bit painful, and not for the wolves.
After a bit more testing, situation is that there does seem to be a 'safe' radius where Geralt puts away his weapons when enemies get a certain distance from him, without input from the player, regardless of when you pulled the weapon. Problem is that this 'mechanic' goes off at the wrong times. I've had wolves jumping at my face when Geralt decides to commit suicide by sheathing his sword without any input from the player (me). I've also been in the middle of a large drowner fight and had the same thing happen. The fact that it's happened more than once tells me that this feature is faulty at best, laughably broken at worst.

It's a poor design choice because it takes agency away from the player and gives it to an unreliable and poorly designed background script that's often faulty and definitely detrimental to normal gameplay.

Bottom line is that this feature is not needed and should be removed. If you walk or ride into a town with a weapon drawn, people freak out screaming. Kind of a great way to remind you that it's not polite to walk into a civilized area flashing a weapon.

CDPR, please remove this or give us the option to turn it off.

Agree. As I said earlier, this feature also make figting archers really hard. Arrow deflection (if you have the ability) works only when you are blocking and if they are out of radius you cant block because you are not in combat mode.
I posted about this in suggestion thread, lets just hope someone from CDPR will read it.
