I just noticed there is a new patch 1.10->1.11 but there also is an updated 1.11 Hearts of Stone and Free DLC downloads available. I already have 1.10 and Heart of Stone 1.10 installed. Do I need to download only the 1.10->1.11 patch or all of it again (free dlc + hearts of stone + 1.10-1.11 patch)?

It wouldn't hurt to download and install all of it. The 1.10 to 1.11 patch seems too... small... to patch the main game, the free dlc, and the expansion. Better safe than sorry.

I would. If They have put updated versions out, it's probably best to install them :)

The patch pathes the game to 1.11. The HoS and DLC versions are there for people who are downloading them for the first time. There is no need to re-download if you already have them installed, and have patched the game -- they do not patch the DLCs separately.

This is wrong.
The 454MB patch from GoG.com only patches the base game. It does not change the dlc package, nor the Expansion.

Galaxy may do, or it may need several operations - don't use it to be able to check,

If working with GoG downloader or other downloads directly from GoG.com, then you need to update all three separately.
I did as You suggested. Downloaded and installed it all. But I must admit that's stupid I need to redownload 5GB just to patch the expansion. I don't use GOG Galaxy and small patches would be most welcome.

I read the patch notes. I also installed patch 1.11 alone, and checked file versions for the dlc in ep1. All were still set to 2015-13-10. Some of the reported fixes confirmed by myself and others to not be functioning (specifically availability of items at Ofieri merchant).

Downloaded and installed the updated 2.0.0.41 Ep1... and the fixes were present.

I think if you do it through Galaxy it patches everything up to date just by updating 1.11, but this *doesn't* appear to be the case with manual patching.

Feel free to do it your way, but I don't think it will work.

It does work manually: I don't use Galaxy, and never have... never will. My game is patched and working as per the patch notes. There is absolutely no reason why they would patch DLCs separately. That's insanity.
I jumped on the forums because I wanted to know why the 1.11 update is 454 MB, but GOG Galaxy downloads 3.3 GB to apply the same update. At first I thought it must be updating all the DLC as well, but now I'm not sure. Why are Galaxy updates always so much bigger than the regular downloads? I thought it would be convenient having Galaxy handle everything for me — I come from a Steam background and am used to things "just working" — but now I'm thinking manual is the way to go until GOG gets this whole Galaxy thing ironed out.

Galaxy doesn't download the patch files as such, it downloads all the files it's going to change and then moves them into the game folder. I think the 3.3GB size comes from patch+DLC+HOS - they all get updated.
When I try to install patch 1.11, i tells me :

"The Witcher 3 Wild Hunt patch log
Error adding: D:\The Witcher 3 Wild Hunt\content\content4\streaming.cache"

And "streaming.cache" is nowhere to be found.
I also can't install the 16 free DLC's : it tells me that my versiion of the game is not updated...

Can someone help me, please ?

P.S : I already installed patch 1.10.
