Where is it, I've explored "most" of velen, playing on hard so avoiding the lvl 20+ stuff I've found. I'm lvl 17 in the "Free City" atm, and I find it kinda hard to believe I haven't come across it yet. Is it supposed to be in Skellige maybe?

i don't remember where it was but it is somewhere in Velen/Novigrad.

The drops are random, so you won't find it where I found it. Anyways, you'll get dimeritium bombs in one of the main quests, but they aren't that useful. I only used them against one or two bosses.

hmm i wasnt sure the bombs and such diagrams were random, figured they'd be manually placed so as to make "sense". Kinda disapointing but I guess it explains how i got 50+ hours in wihtout that bomb showing. Hate RNG design. Oh well the kids want open world random games i guess.

You can visit the Herbalist just outside of Oxenfurt to the North West (hut after the bridge) for any Alchemy diagrams that you are missing.

It will cost you a bit of coin but it is well worth it.

P.S. He also plays Gwent with you and directs you to a master Alchemist in Skellige Isles.

For the record its North East, its that little building with a wall surrounding it. Its called 'Herbalist's Hut' and has its own fast travel signpost. Costed me 223.
