I played both Death March and B & BB.

The one is a complete grind to keep health up and the other is a walk in the park.

No happy hunting grounds in between.

... "normal" enemies are roughly your level, mostly a bit more

You could try looking for mods. Afaik there are various rebalancing mods for Witcher 3, some (many) of them increase the difficulty (or offer a finer grained difficulty)
Mods should help you find the better solution for your needs, if you aren't comfortable in Death March. I just googled this:

Also, there are mods that gives you more ability points per level or makes stuff cheaper so you don't run out of money.

However, DM is not difficult at all once you get used to it, and health regen is absolutely not an issue. You find food everywhere, and 80 HP/sec for free is crazy!
That's why I installed a mod that makes food very less useful, expecially in combat:

So, my advice is: do not install mods that make DM easier, because you'll get used to it and maybe you'll find yourself installing mods that makes it even more difficult! :)

I was fighting all wrong. Blocking instead of dodging (really OP) - everything is like easy again.

Oh I see! Dodging is the best way to deal with monsters, while agains human it usually works parry-and-riposte too. Blocking, instead, is more like a last resource if multiple humans attack you at the same time. Have fun!
