I die so you don't have to... Anyway, have you guys and gals run into any extremely unique deaths in the game? And by unique, I mean deaths besides getting slapped around by scary monsters.

I've died during Roche's interrogation and to the town guards of Flotsam. This is my second playthrough so I don't mind exploring new ways to witness Geralt perish.
I noticed if you get caught in Henselt's camp, when you try to sneak into it, you get arrowed to death like if you refuse to pay the fine for killing city guards. I think you can also get killed by Ioweth's elves after defeating the arachas if you piss him off enough.
I got harpooned by Loredo. Wasn't fast enough for the QTE. It was kind of horrific to watch.
i was running around to heal while fighting the queen on hard and i caught by a snare trap set by Cedric. I was about one grapeshot away to kill the queen by the time. Also, if you try to enter the mist before lifting the curse in Act II, you die instantly after one single blow.

how do you end up getting harpooned by him? I have to play again to get this variation to see it myself

It was when Roche has you sneak into Loredo's house. After you find the key to unlock the door to Loredo's room, there's a cut-scene with Ves telling you it's a trap. There's a QTE right after, and Loredo bursts out of another room with a harpoon. It scared the hell out of me the first time. :p
Failing the QTE with Loredo's mother is truly horrific.
I was kinda hoping that when you get killed in Henselt's camp, Geralt would go kinda beserk like Boromir in LOTR when he got shot full of arrows... but alas. Still, it was pretty cool
I think it'd be safe to assume that most of the unique deaths are the result of failing a QTE. My personal favorite is at the end when you fail the QTE involving the dragon.

Have you ever given a small, crunchy dog treat to an overzealous dog? Yeah, it's a bit like that.

Was so terrified the game was going to make me do the whole fight again similar to the penalty you get if you fail the Kaylan QTE. Was so, so very happy when that was not the case...
