I accepted the quest from the guy and got the real formula but decided not to give it to him. He got mad and stormed off, and confronted me with a group of thugs in the forest and I killed them all. According to the game guide this is not one of the possible outcomes as they are supposed to capture you. Now the quest says its completed but when I finish Act 1 it pops up as quest failed. Also, the guy is still in the forest in the same spot taunting me but I cannot interact or attack him at all. I know he supposedly has a cave he takes you to so I tried finding that to see if it fixes the quest but I cannot find it.
I have also this bug, but in my game I gave it the real formula, received the reward and completed the quest, but it "failed" when I reached the point of no-return in Chapter 1

I also had this quest fail bug when I completed Suspect: Thorak and the walls have eyes in act 2

Yeah, this happened to me as well. I defeated the ambush and the quest failed at the end of Act 1. I hope they fix this soon.
It seems like the only way you can successfully complete it is to be blindfolded. My first play through I agreed to be blindfolded, woke up in a cell in their cave, broke out of the cell, killed them all, and completed the quest. It did not fail at that time.

This time I refused to be blindfolded, went to the hideout, collected the reward, and it showed the quest completed. Then it showed as failed as I was ending act 1 and heading to act 2.

There are several possible outcomes, depending on how you handle the merchant and how you handle the quest giver. Apparently it only ends up successful if you do it a certain way.

Both times I got the real recipe from the trader.

This is the first part of the journal entry, which is the same on both runs:

"Do you like the scent of incense? Then Listen. Geralt met a scholar at the harbor. The man claimed that one of the local shopkeepers sold incense producing poisonous smoke. He could not prove the murderous trade without producing a recipe for the incense. Geralt promised to get the man a recipe for a tidy sum of orens.

Do you think getting the recipe was difficult? Not at all. Geralt brought the information to the scholar at the harbor.

A good scholar takes nothing at face value. After the witcher gave him the incense recipe, the scholar from the harbor wanted to test its authenticity in his laboratory and agreed to meet the witcher on the market side of the gate."

This is the rest of the entry for the successful quest:

"The witcher agreed to have his eyes covered and was led to his employer's hideout. Thus he walked straight into a trap. Yet it was a trap set by amateurs, and Geralt was a professional.

Geralt dealt with his employer and the others. Science was unharmed, however, because they all turned out to be fisstech dealers and the remnants of Salamandra, an organization which had crossed the witcher long ago."

And this is the rest of the entry from this run, which failed:

"Geralt laughed at the notion of covering his eyes. He demanded to know the location of laboratory and brought the recipe there.

The scholar was not who he claimed to be, but it made no difference to the witcher. He knew he gave the man a recipe for fisstech, caring only about his reward, he left content."

Take the direct approach.

1) Refuse to be blindfolded, and be stubborn about it.
2) Go to the lab -- fully armed -- as marked on the map.
3) Hand over the formula and get paid.
4) Exchange conversation which ends something like "The truth will out."
5) Draw your sword. They draw too.
6) Leave no witnesses.

Hmm. I'm pretty sure I said number 4, and I did draw my sword, because I wanted to kill them, but it would not let me attack. I even walked out of the area and came back. Nobody would talk to me, and I could not attack them.
I think you're suppose to fail any quest in which the quest giver is killed. So the bug is that it should not be shown as a "success" when you kill him, but rather failure. I think there are a couple of other quests that are setup this way.

This is mentioned in the game guide i believe (which is outdated though).
It is bugged.

The first time, I refused to give the formula, got jumped in the woods, killed everyone, and got a message saying "quest completed". Then , as I moved to the next chapter, I got a "quest failed" message.
My impression is, you fail the quest if you don't get the exposition that they're remnants of salamandra.
Obviously, if they just attack you in the woods, you won't find out squat.

I go with giving them the real formula at the lab, refusing to work with salamandra, then killing everyone.
That way the quest doesn't bug out.
Is there any other issue than "Journal does not look good with a failed quest" with this? If you get your XP or reward then it should be of no concern.
it hurts the feeling you know. Kids loves to see themselves the achievers.

anyway: you kills the guys and never knows for sure if the formular are real fisstech, and they are salamantras -> quest fails.

for Thorak and the walls have ears etc : Stennis are the killers, any conclusion that lead to he living -> quest fails.

simple as that. Regardless how you end, you get the xp.
