I'm trying to determine what the 16 different ending are. Currently I have figured out 12 of the 16. They are as follows:

Act 2 refers to who you sided with in act 2.
Lodge refers to who exposed the Lodge's plans. If it's Letho the Lodge is opposed by the kings, if it's Triss the Lodge is established.
Anais going to Redania is the default because if you don't help Roche she goes to Redania.
What happens to the dragon (Saskia) is more important in Iorveth's path because it affects who will rule the Pontar Valley. In Roche's path it is always annexed by Kaedwen so the dragon's fate is considered the same choice.
Unsure if Stennis tries to conquer Pontar if he's alive and Saskia is dead, or whether it submits to him. I do know that he tries to conquer Pontar if he's alive. What happens to Pontar if Stennis and Saskia are dead is not mentioned.
The problem I have with the last 4 endings is that if I add another choice to Iorveth or Roche it doubles the number of ending from 6-12, resulting in a minimum of 18 endings. Does anyone have any advice regarding what the 16 endings are?
