To my knowledge, nobody has made a decision map yet.

That being said: Finish the prologue. I doubt that Foltest knowing the attacker was a Witcher will be a big issue.

It functions as more of a "heads-up, decisions this way" warning than anything else, but it also helps you start creating your version of Geralt. Also, Triss doesn't tell you not to say anything, she just says that she hasn't. You won't wreck your game with one choice or the other if that's what's worrying you. Enjoy!

But thats the ONLY option in the entire game that can do that.

Would that actually end the game or what?
And what are the other two of the four possible beginnings? The two I can think of are depending on whom of two related people you save. But... I didn't encounter her where she was in the trailer videos. I assumed that was because they thought they needed to remove the nudity. Or is that still one of the beginnings? I didn't find any trap door there...

No, the scene from the trailer is still there. It's a matter of which path through the dungeons you choose. I missed it my first time through as well.

Turning Roche down doesn't exactly end the game, but something else does. I enjoyed it, and you can reload. Check it out if you have time. It's a great reminder that choices have real consequences for the characters. No one's invincible in this world.

Hm... I was actually in a torture chamber and killed two guys, but there was nobody else. And I've also seen that crack in the wall I've seen in the trailer, but there was nothing behind it. And I ran through that dungeon back and forth many times, no trap door. And would it be too much of a spoiler to say what the 4th way is?

The 4 ways the Prologue can play out depend on two things... Your interaction with Aryan La Vallette, and the path you take through the dungeons. If Aryan survives your first encounter, you then encounter him in the dungeons. If he doesn't survive, it's his mother you encounter. Depending on the path you take through the dungeons, you encounter them in different circumstances... if you take the 'high' path, you come across them being led out of the torture chamber, and if you take the lower route, through the hatch, you encounter them in the torture chamber, and fight the executioner. Even if you take the high route, you have to fight the Executioner I think, to get a key. So, the 4 ways are two encountering and escaping with Aryan, and two with his mother, one each when they are being tortured, and one when they're being led out.

Also you might not want to insult Iorveth when all his squiirrels have their bows pointed at you :P
