There a particular reason you chose the heart for the first version and switched it to the silver sword diagram? Seems to me that the best item to choose would be something like the swallow formula since you have to actively put it out of your inventory to get rid of it or have your inventory stripped when captured a few times in game, thanks to you starting the game with that diagram and it's unlikely any player would want to drop it since it's a pretty great and simple potion.

Players, while a bit unlikely, still may skip the sword diagram, especially if they import since you only lose the unique transfer sword when when captured and may just ignore the quest and chest entirely with foreknowledge. Then again, that takes in the fact that the player is aggressively clicking any red-glowing chest in the vicinity.

Like I said above, I am reluctant to want to stick the effect on a mundane item because it could accidentally be sold and the buy-back price could under some circumstances be prohibitive (The Swallow Formula would sell for 15 but cost 345 to buy back once it left your inventory). Multiple copies of the formula might stack in effect or re-break things like the Haggler perk/ability. If I made Swallow Formula a quest item so it can't be lost then you may acquire multiples without being able to sell them and effects could stack again.
I appreciate the suggestion, it's a good one since everyone starts with that item, but there are other drawbacks and for the moment, unless you're already past the first act, the schematic can be found at any time in a crate with red dancing particle effects all over it screaming "I'm important! I'm a quest item! loot me! loot me!"
I am open to the idea of changing things if needed and that could include using the Swallow Formula at some point if I can be sure the possible issues I've mentioned won't be a problem.
Also, is the Swallow potion superseded by anything better later on in the game?

I believe Gadwell may be better if one goes down the alchemy path to make the negative side affects weaker
