I've been having a really annoying problem during combat where every now and again the controls stop working properly.
I can't turn the camera or attack or do anything, literally the only controls that still work are the movement keys and the pause menu.
It's extremely annoying since it completely screws up combat as theres no proper way to get the controls back to normal.
I've tried many solutions. Saving and reloading leaves the controls stuck as they are. I've tried attaching a gamepad when it happens and that solves nothing. I've looked everywhere and no one else seems to be having the same problems I am.
Is anyone else having similar issues and able to suggest anything that could help.
Game is fully patched btw.

There doesn't seem to be any fix as of yet, but several CDPR employees are on these boards and watching people's problems so hopefully it gets fixed soon.
Google is your friend. There are lots of reports of controls going kaput. It's a known bug. Just have to bare it out and hope 1.3 fixes it. Until then, save often
This only ever happens to me if i use an item such as knife, bombs ect and it turns out i didnt have any of em even tho the icon shows i got it equipped in the lower right corner.

Have to reload a previous save to fix it.

(mouse and keyboard, dunno about controllers)

This. Exactly my issue - you run out of an item despite the icon showing it as equipped and the controls will just die on you. Pause works and movement controls work, but not anything else.

I love the game and all - but it's a little annoying. I've had it happen three or four times in the course of my entire playthrough.
This seems to be a rather common problem. Have a look at my List of known Bugs for links to several other threads. I'm afraid there is no known solution or workaround so far, and no fix has been announced yet.
Yeah, I'm pretty sure this is top-of-the-list annoyance for everyone no matter how much they might complain about the QTE being invasive or the game being too difficult in the beginning and/or too easy in the middle, or certain narrative elements not making sense, or even the fact that you have to place yourself in just the right spot in order to be able to pick up something or use something... and preferably unarmed. (It might be my imagination, but it seems loot is a little afraid of swords.)

...It's especially annoying when you're caught in the middle of a, say, 10-scene battle that doesn't allow you to save, and suddenly your keyboard stops working, or your mouse stops working and you get killed and have to start all over again. ...Or, say, Zoltan gets caught in the middle of 2 scoia'tael and just keeps running in place, you run back and forth to try to get him to respond, you bump into him, into one of the rude elves... finally the only thing that gets him loose is dodge-tumbling into him... odd.

But it actually seems to be much more common than random occurrences of Geralt being unresponsive to the mouse or the keyboard... there're a lot of times when he's free of anyone attacking him and he simply will not cast a sign -- for just two or three precious seconds -- or throw a bomb . . . and times when he's in combat and for some reason will not dodge. It happens so often it's ridiculous. It's not at all just occasional, but the thing is... when it does happen it's usually fleeting . . . for some odd reason: at times the game just doesn't respond right away.

So... no, Jingafex, you're not the only one.
One thing you could try for a workaround is to uninstall the manufacturer's drivers for your keyboard and mouse, together will all tools they bring along (e.g. Logitech Setpoint), and use windows' default drivers for keyboard and mouse instead.
