I'm sorry, I have to vent right now.

I just strolled back from the Melitele's Heart quest, adorned with a nice new piece of jewelry, as I arrived at the waterfall and promptly, a couple of drowners surfaced. After I made away with them, I barely believed it, but one of them had a madness mutagen with him! With shaky hands I skinned the beast and prepared to rejoice. But first, a quick save, lest I crash and loose the coveted trophy again. Alas, I shoulda done a proper save, because of course I pulled a ctd by F5. Argh! Madness!!

Of course reloading and doing it over a dozen times didn't help. Argh!
What was that saying? Doing something over again and again in anticipation of a different outcome is the definition of insanity or something like that? Madness!

Has anyone ever found a madness mutagen in Flotsam? I thought I'd heard those are only to be found at a special place in Chapter II? I didn't get any there on my first run, though.

I don't think I've ever found one in Flotsam. Seems like it is a random drop (I remember once it dropped from a harpy in Chapter 3) and I don't think there are "special" places in Chapter 2 for those.
You are sure you saw it in the loot window though? It's possible that it is just an extremely rare drop rate in ch.1 and you lucked out, so to speak. Is it possible to proc them from alchemy? I had a madness mutagen in my inventory during ch.1 but I don't remember how I go it, whether through loot or by mass producing potions and bombs with the 10% mutagen proc talents.
Ouch that sucks :(

I had one drop in Ch1 in my last playthrough. Must be wayy less than 1% chance if in 3 playthroughs I've only gotten one in Ch1.

Yeah, my save at the end of chapter 1 has a madness mutagen, though I don't know where I picked it up.

Probably while practicing weird sword moves by respawning nekkers in a cave (Malena).
'cos I killed hundreds of those things.
Yep, every time I meditate first thing I do is go back to that cave and clear it out with some dancing stars.

I'm not sure if it is possible to make madness mutagens as side effects. But I do know, that I have a convenient diagram in my pocket, for crafting a madness mutagen, once I have enough raw materials (Mutageneology mod). That's something.

Plus I've learned my lesson: "when you are afraid you might crash on F5, because you just got lucky, DO NOT quicksave!"

Yes, to be safe, use regular save!
I did read the title ;) wasn't sure if you were second-guessing yourself if you had really seen it or not before you crashed. I am going to pay attention to my mutagen procs from now on. I am going through on insane and taking my sweet time lol. I have arm wrestled almost an entire level out of Big Max.
I feel for ya, mate.

On my first playthrough I got 2 Madness in Flotsam. I haven't even smelled one anywhere since I "upgraded" to 1.2

They are simply a rare drop - and you can get them anywhere - but only if you're lucky.
Think I might hVe to check out this mutagenics mod you mentioned, I am guessing it allows you take take lesser ones npand combine to regular, and regulars to greater? I must have sold over 150 lesser mutagens so far...

Without this mod, all those masses of mutagens in the game make absolutely no sense. Who in their right mind would buy them, if they are not a witcher or deal with other witchers regularly? Bad design flaw - but this mod remedies that. :)

Was I really that unclear in my narration of the events? Or did you really only read the title? I mean, how would I have lost a previously present mutagen? I save all the time because of the crashes...

For me, they came from the drowners at the falls , the neckers in the first cave under Flotsom on the second tier near the back, and the neckers near the hospital ruins. Hope that helps. Those mutagens sure are difficult to come by, so I hope you find one. ( your results may vary )

I've never used one myself, but apparently it also adds a bit of sword damage.
