I think you have misunderstood something. From the little I saw, he is talking to a friend live while streaming, and the characters of the party are either based upon other people/friends or are other people's chars from other cRPGs / p&p RPGs (not sure).
The game has no official or unofficial support for multiplayer, although it could theoretically be done by sharing the desktop with some remote access software (if you really expect everyone to behave).
TLDR: The only one actually playing the game is him.
