The game starts in windowed mode whatever settings I set to get fullscreen mode. Must change the video setup every time.

It's a work-around for a crash that happens on some setups when playing fullscreen, that should be disabled once Failbetter find a real fix.
I agree that it's inconvenient, having the game in windowed mode only on first launch would have been enough in my opinion.
