I'll do this a spoiler free as possible.
During a story mission (investigate some strange energy), I gained a special action to help me complete the mission. However, I saved before attempting something dangerous. Upon loading that save (didn't go as expected), I can no longer use that special action. The UI element is still there. The hotkey and the mouse do not activate the action.
On a side note. Man this game is good.
