So, I've been struggling to get exactly one item for each of the void psyker, force psyker, cyberninja and the prototype. Looking through the forums here and on steam, I learned that these items are obtained by completing certain actions. However, even when I perform those actions, I don't get anything.

For void psyker, I read that it has to overload in order to create the "Essence" . I tried doing that many times, even tried doing so when the unit is debuffed by madness.

For force psyker, I read it has to drop to zero force energy and then I get the "Glowing rune". However, I also read that it's broken.

For prototype, I read I get a "chaotic haiku" when it malfunctions. It does read out that haiku, but nothing is dropped/added to my inventory.

For cyberninja, I need the "forging nanites" or something, but I don't even know how to get them.

Any help on this would be appreciated.

There is also a probably related bug where my inventory suddenly shows I have more items than I can see: e.g. I see 40/40 when there are 39 items, and I can't add another thing. This goes away if I save and reload.
