I've just purchased the game, my first observation was there are no speech during in-game dialogues (when you talk to other NPCs), only subtitles. Why? The voice acting was top notch in this game when I played the cd version some years ago.
This question / problem has been solved by Firekimage
yeah it seems the voice isnt working again. it DID work for me a couple of weeks ago when the new installer was released. i uninstalled after beating the game. downloaded again couple of days ago, and sound isn't working now. did they revert to the broken installer?

I can confirm this too, redownloading the installer revealed that it contains the original executable again, in which there's no voice in dialogues.
It might be a good idea on the part of the support placing the fixed exe into the downloadable content of the game on the my games page.

Confirmed here as as well - replacing the exe with the one from page 1 produces voices... but the directly downloaded one does not.
Raising a support ticket for someone to look at this tomorrow.
Not had a response from support yet about this....

Hi I bought the game in the year 2010 and met the very same difficulties. I have request the support team as well.
According to this topic, the problem was solved despite the issues are not :-)
Can you advise if you still have this problems with you now?
Anyone, please post here if you still have no speech as well.
Downloaded the game today (8th Jan 2010) and the game dialog didn't have sound till the replacement exe was applied. Just checked it for a short while so I'm not sure about stability, seems ok.

Sorry guys for all the trouble, we were doing some upgrades to our hamster-powered server farm at the end of last year and it seems that Sanitarium setup file was mixed with the old one ;)
Won't happen again!

This is why I never use hamsters on my server farms... you just don't get the reliability of gerbils!

I downloaded the game today (10th Jan) and the dialog sound is still not working.
I am running under Vista 64-bit.
Same here. I downloaded it today and have no dialogue sound as well.
I will try the fixed executable in on the first page though...

The game was purchased on 28 Dec 2009.
First install attempt on 07 Jan 2010 results in no speech in dialogs.
Second install attempt on 08 Jan 2010 - same result with no speech.
Third install attempt on 10 Jan 2010 - no speech.
Last attempt today (just downloaded the setup_sanitarium.exe [1.2Gb] again) - no speech!
Two questions:
1. Can it be checked by GOG.com officials: that the setup_sanitarium.exe file is correct at last?
2. Dokole?
You can ignore the second question, but please make sure the first one is to be SOLVED PROPERLY once and for all.

There are two possibilities left:
First: the new file hasn't been propagated throughout our file hosting provider's mirror server network as it should, which may be why some people are still downloading the old version.
To verify this I will need your help. Please check the exact size of the downloaded setup_sanitarium.exe file.
The NEW version has: 1 235 894 636
The OLD version has: 1 236 065 614
Second: The setup file has the correct size but the sound still isn't there. This might be caused by the remains of the previous installations.?To fix this, you have to completely remove the old Sanitarium installation and install the new one in a different folder than the last time. Then launch the game from the start menu shortcut and NOT directly from the game's exe file.

I tried it yesterday in the middle of the day and got the old file (1236...). Unfortunalety I erase my download history, so I can't tell you the servername. Actually Free Download Manager shows me the correct file size (1235...) and it seems to be the correct file. Servername is wpc.11df.edgecastcdn.net. I will give it I try later this evening.
Edit: Short game test, after reinstallation I just loaded an old savegame. Talkie works now, so thanks from my side.

It seems I have got an old one, the file size is exactly 1,236,065,614 bytes.
According to my download manager history the server address was: http://wpc.11df.edgecastcdn.net (or just edgecastcdn.net )
It is equal for all my recent GOG.com downloads (other games are included).

I now uninstalled the old version and downloaded newest version of sanitarium. Voice now works perfectly. There are still random crashes though
