I tried replaying the part where Ed had to key in a 4-digit code to open the door to rescue Dr Morale, by purposely neglecting to pick up the journal. Strangely in the hospital Ed still managed to produce the journal for Anna in the hospital later.

Was I mistaken or is this a bug?

No, it's not a bug - he picked it up anyway.
