What do you think happened after the end of PS:T?
If you got the best ending, Fall-From-Grace assures you that she will at least try to track you down in the endless battle, that is, if I remember correctly, somewhere in the grey wastes? And she seems to be quite confident that at least theoretically, she is able to do so. I think, she says that TNO has to remember her as long as possible to aid her finding him. Also, the rest of the party (except Ignus and Valhor, who did not make it to the end when I played the game) seem to be willing to support Fall-From-Grace.

So - what then? And what happened to Deionarra after the Fortress of Regrets collapsed?

The rest I don't know about. Presumably, she passes on after saying she will wait for you in Death's halls; Deionarra's Truth restores more content that suggests that she is being held there by here passions, namely her love for TNO, so unless you tell her that you don't love here, she apparently is still just kind of... stuck there. Forever. You monster.

I do not exactly remember... why did she have to stay? Was it for TNO to maintain a connection to the fortress?

TNO (or rather, the Practical Incarnation) killed her to make sure that she stayed anchored to the Fortress to be his eyes and ears there. As for why, it turns out that to an extent, the Dustmen philosophy is correct; spirits cannot move on to the next life if their passions are keeping them from doing so. TNO had counted on Deionarra's love for him keeping her at the Fortress and preventing her from crossing the Eternal Boundary.

Ah, right, thanks. Why then can you meet her at the mortuary in the beginning? Is she able to travel the planes somehow?

Probably it's some magical connection to her tomb at the Mortuary. I don't know, that wasn't explained in great detail.
Yeah, there's no way around a certain amount of speculation dealing with this topic...
What I find far more interesting is that the Blood War is supposed to be Eternal. It ended in 4th editon D&D though. TNO may well be free, having served his penance :D.
