It's an old game, it won't run in fullscreen mode on it's own. You need to get the Widescreen mod over at Gibberlings Three and install it.

One of the many reasons I won't upgrade to Windows 10. It plays in full screen in Windows 7 without special tools or 3rd party downloading.
Thing is that, for me in win10 even without that full screen mod, i can't make it work in full screen...
It's pretty annoying to make it work properly on Windows 10. You must install all mods and sometimes it keeps crashing

GOG version Works splendidly here, Win10x64, full screen, all mods installed, and running at the custom res of 1400x1050, which I prefer for this game to my native res of 1920x1200 because things are bit too small and some detail disappears at the higher resolution. 1400x1050 with screen fonts & UI @ 120% of normal is perfect for me.

It's pretty funny because now it works flawlessly without any mod...they must have fixed it.
