There is a pack of ogres in the in the northern Elmshore area.
Two of them are druids and they would cast plague of insects which would stay permanently on you and your party. And the worst part is that the effect seem to be stackable. My entire party had two debuffs at once.

After somehow surviving the fight, i noticed that the effect isn't wearing off. Out of combat regeneration would mitigate the endurance damage, but not the health damage. Even resting doesn't remove it.

Nasty bug which needs to be squashed!

I haven't got to that part yet, but my party is experiencing a similar bug, ie. when rested with bonuses (at taverns etc.), the bonuses do not go away... they are permanent. They are supposed to wear off after a time, but they do not. This game is great, but at this stage it's as bug riddled as any game I've ever seen.
Hmm, i am pretty sure that after you rest in a room which gives bonuses, they stay until the next time you rest.

Really? So you get a 'rested' bonus for potentially days? That makes no sense at all.

Edit: no, this is not the case exactly. In this post by Josh Sawyer, he states that resting bonuses are temporary; they last "for a specific number of rests following your stay at the inn".

Well i am not sure the bonuses go away if you get fatigued. Actually i can't remember if that is the case, but, they do otherwise stay till the next time you you take a rest.

I encountered the same bug during the same bounty quest with the Ogres. :D

I was going to despair, BUT then I noticed that it does go away, if you close the game and open it again. Yes. Reloading does nothing, but completely ending the game process does it. :)

Definitely going to try that. =)

I just came across this bug, too, and saving/reloading does make the insect plague go away, BUT my party now had a permanent -20 concentration penalty, and no amount of resting would get rid of it. I determined that removing party members at the stronghold, and re-recruiting them got rid of the penalty for them, BUT (again) you cannot do that to your main character, so now my main character is permanently crippled! Damn, this game has some serious -- and I mean SERIOUS -- bugs.
Yes, this issue has been reported on the official forum too.
It looks like there's no sure way to fix it if you are affected.

Interesting bug. Saving/reloading DIDN'T make it go away in my case. And I didn't have a permanent effect either - after I ended and restarted the game.

Go into the inn and use common room to rest. That should remove the buffs from previous rest.

Yes, but everyday travel and conflict should quickly remove buffs. To have them stay until you have rested several more times is just utter nonsense.
There is actually another bug i experienced.

My cipher stopped regaining focus with soul whip. Why this game must torment me so?!

Yeah. It is stupid and let's hope they fix it. It was my understanding that we would lose rest benefits after resting again. It is not so. Maybe the amount of rest is tied to the level of difficulty, like camping supplies?

That one seems nasty. How did it happen? Luckily my Cipher works fine so far. Knock on wood.

On the contrary; this is rather intelligent.
Try going camping for a week straight and notice that you don't get all that rested, compared to sleeping in a cushy bed, with beers in the fridge and bandaids in the cubboard.

Traveling is HARD; resting at a comfy INN means you get the benefits of all those luxuries and just like nutrition, a good night's sleep for one night can improve your next few nights, even when those following aren't the best nights sleep.
I'm guessing the majority of the benefit is derived from the nutrition you can get while there.

The benefits wear off one bonus at a time, per rest; since there are only two bonuses per rest, this means two rests to get rid of them.
They are also immediately replaced by any new one you get.
