I am looking for help as my game is stuck on 640 x 480.
I'd like to run the game in 1024 x 768

I have tried many combinations in the compatibility settings, Including maximized, windows, DPI, 16 bit color, etc.
And yes I tried reinstalling. I also looked in the files to see if I could find a notepad resolution settings without luck.

Please help I've been trying to fix this on and off for 6 months now.

This is how my screen appears:

basically I'm missing 1/3 of the left side when the game runs.

Disable User Account Control, as it's placing the settings into a virtualstore, which the game is not recognising so your settings don't "stick". UAC is a useless feature and I've had no issues using my PC with it disabled.
My advice would be to temporarily set your desktop to a low resolution (1024x768 if it supports it), then launch a new game in Pharaoh. From the running game you will have the option to set the game resolution to 1024x768.
Then you can quit it and reset your desktop resolution to its previous value.

(the switch to a low desktop resolution should avoid the missing part of screen you get)
UAC made no difference. It still starts in 640 x 480.

I am a weirdo and I already run my computer in 1024x768. 800 x 600 had no effect either.
I also switched Nvidia to let me decide, nodda, nope, zero, zilch.
