Do you think? Or does Shadow of Chernobyl win? Or is there some even more obscure game I haven't played?

Pathologic is pretty obscure, but I think Vangers goes a few more steps towards utter madness...
I am pretty sure such bizarre game couldn`t be created anywhere else than Russia.

Well THAT sounds like something I'd be interested in, then!
There is also The Void and Knock-Knock from the same devs (Ice-Pick Lodge).

And don't forget about Space Rangers.
It's not obsure or weird, but awesome in many other ways.

Pathologic references a lot of literature and a bit of folk philosophy. Even the western literary influences are seen through a Russian lens. It has a lot of scope which allows more of the authors' worldviews, attitudes, and influences to come across.

That said - I'm sure not all Russian towns are like the town, and not all Russians are like the game's developers. However, it isn't a game that could've been made anywhere outside of Eastern Europe.
