I just started playing World of Xeen and I'm a bit confused on how you are supposed to play it.

Seeing how both MM4 and MM5 are self-contained games and each are made to be played with starting level 1 parties, how are you supposed to approach the combined game in World?

If I progress to level 20 in the MM4 world then go to the MM5 world, won't 20 hours of content in that world be a total cakewalk? And then if I spend 20 hours more in the MM5 world and go back to the MM4 world, won't the same thing happen, all the quests I need to complete will be have content that is extremely underleveled?

Should I be constantly bouncing back and forth between the worlds as I play?

Don the MM4 part first, then the MM5 part, and finally the part added for World of Xeen.
Darkside is balanced with the assumption that you'd play Clouds first.

Early on in Darkside, there are some large XP awards, to the point where it won't take long for your levels to get rather high. In particular, this is enough to break the balance of Clouds; therefore it is recommended that you play Clouds first.

Because of those high early XP awards in Darkside, most of Darkside *expects* to to have those levels, and therefore having them isn't going to make Darkside trivial,

The general rule of thumb is, if you're planning on playing both worlds, is this:
* Play through Clouds first. Until you defeat Lord Xeen and get the ending, don't use any of the pyramids.
* Then, use a pyramid and play through the Darkside.
* There are some items that require going back to Clouds after you find an item on Darkside, Do note that these dungeons are balanced for that. Also, note that the Dungeon of Death should be treated as a bonus dungeon; it's not required, even for the combined ending, it's dangerous, and you don't get a reward that would help you complete the game. It can, however, be fun to explore, and the final floor of it will surprise you. (Without spoiling it, all I can say is that it is most likely not what you're expecting.)
