I have the exact same issue on win 7 x64. Any ideas?
After launching the game everything is normal. I can play a little, but if I die or load a game, the sound starts decreasing over a few seconds until it's completely muted. Funny thing is that if I pause the sound stops decreasing, but when I'm in game it will continue to decrease, again, until it's fully muted.
WTF?

Update: I think I understand why this is happening.
When you die, the sound slowly decreases until it's muted, and you are then presented with the Load Game menu.
When you load the next session sometimes the decreasing sound will carry over. I couldn't find any reliable solution to this so far.

I have found the workaround. Once sound is disappear in game, press ESC, go to menu -> options-> sound and change volume level). It helps to restore sound in my case until player's death. Also I have turned off Music and set EAX/ DS3D as sound source.
