I've found that attempting to use the settings menu on the main screen results in... a mess. Subtitles, Tutorials and Rollover assist are the only options shown and none can be changed. Exiting the settings menu makes my install switch to windowed mode. Starting a new game after that has low sound volume and possibly a soft-lock after a few minutes play.

Changing settings in-game with the pause function works fine, however.

If you have attempted to use the main settings menu and were unable to change any setting you might have to delete your settings.ini file. I'm unsure which one is corrupted so you should delete Gog\Mad Max\Settings.ini as well as deleting the Mad Max directory under Documents\WB Games to fix this problem.

I suspect it's related to some online function that no longer works.
I just started it for the first time, and it works fine. Not sure if there was an update or not.

Looks like this is a bug that I'm getting too. You can get around it by starting a new game, getting through the intro (which will have no audio) and then changing your settings in game like you describe. Take note that the cinematic volume is set to 0 by this bug (hence the no audio during the intro). Once you have your settings setup as you like, quit out to the main menu and start another new game. Everything should be working as intended then.
