Health regen is displayed as "per second" but is it always real life time or in-game time? Meaning when I set game speed higher will it fasten regen accordingly or I will be disadvantaged?

Higher game speed only effects walking speed. Combat speed stays the same so there shouldn't be any disadvantages.

There are separate sliders for walking speed (up to x4) and fighting speed (up to x2.5), besides regeneration works outside of combat as well.
