Just finished the Iron mine quest and delivered the head of the orc khan. I also have the relic for the promotion to duke quest plus the gold but no such topic comes up with the npc.
The promotion topic had popped earlier but i couldn't finish due to money shortage.
Is it now bugged and won't come up?
Anyone has seen this before?

Have you talked to priest Romius west of the Hillbrandt's church? Maybe that could help.

I had both Romius and his Vampire Liege killed well before knowing there was a quest involving them.

And do you have Romius's confession regarding the Monstrance? It says: "The priest, Romius, confessed to serving the vampire Herbertius...", and it serves as a proof required by Duke Weinberg so that he could promote you. If I recall correctly, you shouldn't be able to kill Romius before he confesses - unless you attack him of your own will.

I don't think you can refuse the quest which means that there could be a rare bug in Weinberg's script. Could you please send me your current save to kovar[@]cinemax.cz? You can find your saves in "Documents\Inquisitor_SaveGames". I will have a look at it - maybe I will be able to find out what went wrong, and I should be able to finish the promotion quest for you or edit the save file so that you could finish it yourself.
