Why does GOG include manuals for only 3 of the chapters? Were the others released without manuals, or is it just that GOG can't get them?

These manuals are almost identical (except Tarnum's letter). Only 6 chapters out of 8 got printed manuals:
Chapter I: Warlords of the Wasteland
Chapter II: Conquest of the Underworld
Chapter III: Masters of the Elements
Chapter IV : Clash of the Dragons
The Final Chapters VII-VIII: Revolt of the Beastmasters & The Sword of Frost (single printed manual for both chapters).

And one more question. Are the levels found under "Preview" the same ones you find in the actual game being previewed? There is no reason to play them if I'm playing all chapter anyway, right?

Yes and Yes. Preview levels were added back in the days as teaser for buying next Chapters. :-)
