I'm stuck attempting to do the Tholean Tablet quest. As soon as I attempt to enter the room with the mallet and gong the room fills with green poisonous smoke, I ran around and got the mallet and all my characters/souls die attempting to do anything else in the room.

Am I missing something or is this quest bugged for me?

No it is a bug that has already been fixed BUT so far only for the Steam version. Very frustrating indeed.

Once you bang thee gong three times, change to the Devourer . You'll be attacked but fight your best and beat the enemies. After they're defeated, walk to the sarcophagus and change back to Evia, or whichever main character you're playing. You'll find a lever inside the sarcophagus. Pull it and the door will open. The room is still full of poison gas, so change back to the Devourer and leave the room. Once you're outside, change back to your main character. You'll also find the Tholean tablet here. It's inside the sarcophagus.
