I was going to say "Planescape Torment", but others will mention it anyway, so I'll mention one that's far more obscure: Embric of Wulfhammer's Castle.

It's a fantasy RPG parody that is mainly focused on character interaction and various sexual encounters. And while this concept could easily ... blow ... the writing is exceptionally good and makes the game work in a very unique way.
Betrayal at Krondor.
It manages to combine quite an open world with very good story telling.
I read the four Riftwar books before playing it, and I must say I actually prefered the characters and writing of the game to the books. Now that is unique.
I'm sure by saying crpg you meant tactical pseudo rpg, so Fallout:Tactics, because it's the first game ever to handle ambushing right.

Hm. Well, I have especially fond memories of Lufia II for the SNES. It was an respectable RPG in its own right, but went a step further by integrating Zelda-style puzzles. A worthy game for any fan of Zelda and Final Fantasy VI.

The SNES was blessed with many awesome RPGs, and one of those was Super Mario: The Seven Stars. That game game broke the traditional mold of RPGs. For example, you had to time your attack and defense moves if you wanted to increase their effectiveness, you could see enemies and move around them to avoid battles, or to make surprise attacks, and the setting is neither fantasy nor sci-fi, as it is something of its own. This game also lead to later Mario RPGs, some of which are pretty darn good. (The Thousand Year Door, for example.)

As for the PC, the pickings are not as good in my opinion, but like most platforms there is something for everyone. In this case, I found that Planescape Torment is always worth a mention. The combat is tedious, but the game has some of the best storytelling in a Western game, and many roleplaying opportunities. Add the excellent atmosphere and setting, and you have one of the rare 9/10 games that are apt to keep that rating.
The world they created in Morrowind deserves a nod.

Drox Operative deserves another...

And of course, the last goes to that bastion of originality: "Lord Of The Rings 'Inspired' World 436."

You are right. I've never met anything close to Planescape Torment.
Fallout and Torment seem to be the best answers.

I'm currently playing Risen 2 and the focus on colonization and such is really unique as well. I'm enjoying it much more because it's not random fantasy or sci-fi.

Yeah, there's a completely separate dialogue tree for it :D
Gothic 1 has a nice unique setting and story, and the controls are pretty unique too hehe

Whats a CRPG? Didn't they just call these things RPG's long ago, why do we need sub genre's of the damned things.
