It is a bit hard to say because party creation may affect the difficulty quite much. I used lots of times to create a "perfect" party, rerolling umpteen times for each party member until they got very high ability points etc. If I had just taken some of the pre-designed characters instead, I presume it would have been much harder for me.
But still, I considered e.g. Baldur's Gate 2 considerably harder, even though I ended up playing BG2 in Normal difficulty. I just remember some demi-liches from BG2 with their instant kill spells, dragons that just blew you away, bosses with very high magical resistance, and annoying vampires which you had to counter with certain two spells.
With Icewind Dale, it was more about just coping with masses of enemies at the same time, not that bad. The expansion though had one very hard boss encounter if I recall right, but after you realized that you shouldn't care about his minions at all, but just rush with everyone to kill the boss himself => cakewalk (the minions disappear when their boss dies).
That reminds me: I always found the Butcher mid-boss, and the Diablo itself, aggravating in the first Diablo game. Mainly because there didn't seem to be any tactics involved, all you could try to do is to keep hitting them hoping that you don't run out of HP and mana before them. I'm surprised I had endurance to beat them.

Humans Must Answer is very hard on normal difficulty. So far the only game I had to give up, because I wasn't able to make any more progress after a certain point. Might retry on easy difficulty in the future.

It depends what you mean by "hardest". Its fair to say that some old Sierra games have a "steep learning curve", but once you beat them once its no problem. And once people accept that saving and loading and dying and clicking on everything is a part of the entire Sierra experience, they aren't
Then lots of old RPGs are easy enough to figure out but hard to master. I've found that most have some sort of balance issue that will eventually make them possible to beat without too much trouble, it just takes some trial and error to figure out what works
Then there's games like Master of Orion. It has a hard enough learning curve, but once you get a handle on things you can win on easy every time. But if you play on impossible you will get your butt kicked sometimes no matter how good you are, especially playing with weaker races. Same with FTL to a lesser degree.
Besides all that, it depends on what you're good at. Some people just aren't very good at action/fps. While I'm certainly not the best, none of those games would be on my list of hardest games. But I understand that they would be for some people.

I suck incredibly hard at Commandos. I was never able to beat the third mission.
