The Dark Mod for Doom 3. If they can just find a more professional name for the mod and release a full campaign, it can easily pass off as an AAA effort.

Reclaiming Sancre Tor for Oblivion. Adds a whole new story to the aftermath of the oblivion crisis. Well worth playing through. You have to have finished the main Oblivion questline before you can start this mod.

I would also mention Nehrim for Oblivion. Great story, improved RPG elements (no min-maxing, like in oblivion) and also taught me some german.

Go to the Total War website and look under the mods section in Medieval 2. It's by far the most popular mod there. Or you can look at Moddb instead.

I think my favorite fan-made mod of all time would have to be EasyTutu for Baldur's Gate. Baldur's Gate Trilogy is also great, but I prefer Tutu because IMO it stays closer to the original game.
I'm also a huge fan of the Morrowind Graphics Extender, and a bunch of graphics improvements of which I think my favorites are Vurt's tree and grass mods.

I'm not sure that I can think of just one.
Afraid of Monsters DX for Halflife 1
The Hidden Source for Halflife 2
Nightmare Squad (I think that's what it was called?) for Doom 2--it basically changed up the weapons and added gameplay elements like weight. And as a result, the entire feel fo the game changes.
LURK for STALKER Shadow of Chernobyl. Makes the game even more survival-focused and frightening.
