Well, there *were* some pretty narrow-minded people in the earlier centuries... And quite a number of religious fanatics still consider homosexuals to be the spawn of Satan. Thus, I can believe it's true, but yeah, we can hope it is not.

Well, no, "cod" meant something like 'bag", so the fish was named this because of its shape, possibly. That's what the "cod" entry says: http://oxforddictionaries.com/definition/english/cod

Unless he was a linguistics teacher, I very much doubt that.
That's got the distinctive mark of something that sounds too colourful to be true, like the believed (and wrong) origins of "fuck you" as being "pluck yew", and many others.
