Wild guess, as I can only connect it to a part of the hints:

I'd probably join everyone else shouting id/3DRealms, after all isn't it the most likely?
However the thing which first jumped to mind was a long shot and that was the resident evil series (yes they had PC ports).
You could probably argue either way over most of the hints, with zombies and stuff and the cannibalistic ways and all.

GOG has stated on Twitter that it isn't Resident Evil.
Anyway, I'm going to say Apogee/3D Realms as well.

Not sure how the clues match up for this, but, Midway definitely needs the cash, so I wouldn't be surprised if we started seeing some of their old games pop up on here and other digital distribution services.

Yes, I know I am very foolish. But... I want to believe.
It is unfortunately likely Apogee/3D Realms. However, if that is the case, I will still be happy for those that will rejoice at such an announcement. Because I know that many of their games carry good memories for many players. It would also be good for GOG.

You might be onto something there. It was developed by Wildfire Studios, which could be what the "flame" is referring to. The "evil" could be referring to Devil's Island, a table that was dropped from Balls of Steel and released as a game in it's own right.
Based on that and the rest of the clues so far, I'm going to have to go with 3D Realms/Apogee too.
