If we're going to be talking about the Half-Life games, I better mention this part in Half-Life 2: Episode 2. I'm going through these abandoned buildings to reach the car, and at one point there's a poison headcrab zombie in a locked closet. It's moaning freaked me out the first time I played through this part, mostly because I heard it before I could see it and because it almost sounded like it was singing.

I also remember playing Minecraft and seeing an Enderman inside my own damn basement!

Yeah, I remember that bit. Ominous, but luckily over quite quickly.

Charon121: Oh yes, I'm arachnophobic too. I stopped playing Doom 3 on the level where you first encounter those spider demons. Just quit the game and uninstalled it a couple of days later. Before buying a game, I now usually check whether it contains any spiders and how ugly they are. If it does, I might be able persuaded to play it, but only if I find a detailed enough walkthrough listing every enemy's location, so that I cannot be surprised/jumped by them.

Incidentally, what game has the worst spiders? For me, it's Thief 1. Such an old graphics engine, yet they move in a disturbingly realistic way.
Skyrim
Inching forward in X-COM, that terrible ambient track gnashing your ears (in a good way) as you progress. Then suddenly, shots at your squad! You see the shots, you hear them, but the enemy remains hidden. X-COM freaks you out with what it doesn't show you, like any great horror.

Oh yeah, that's a good one. That one ambient music track nearly drove me insane.

Dark Messiah - Might and Magic, I'd say. I'm not even arachnophobic, but they gave me the creeps something awful.
My scariest gamemoment was in Star Wars: Dark Forces.
During the Gromas Mines mission, when I set all charges I was unexpectedly pitted against the first generation Dark Trooper.
My heart almost beated out of my chest.

I found Alone in the Dark also pretty scary back in the day and also Doom was rather creepy with those wolfmen.
When I played Wolfenstein on my Apple //e, I always shat my pants when a gestapo officer entered the room (was accompanied by a loud sound effect).

More recent, Stalker SoC had some really creepy moments and an excellent atmosphere that go with it.
The only time I got scared while playing was when I was in a area with mummies or celing claws in TLoZ: Ocarina of time.

The first time I met a bloodsucker in an underground tunnel, that scared the crap out of me.
I have never finished System Shock 2. It was too scary.

Also Alien vs Predator was often scary.

In general, the anticipation is more scary (when game uses music, sounds, lights, etc) than actual monsters.

Project Zero / Fatal Frame series has it all at least for me. It is quite chilling all of those ghosts lurking around and a camera as only mean of action gives a real survival feel. Personal favorite the 2nd game, just re-released on Wii.

I did, but with only four legs, those things aren't technically spiders, and my subconscious mind knows it. ;)
System Shock 2 just had me on the edge of my seat the entire time. it was all the little details like enemy chatter, the dark corridors, the eerie background noises, ghosts, the creepy character animation, and disturbing audio logs. After all these years, this audio log remains my most vivid memory of that game:
