In most games I also tend to try to minimise civilian casualties.
However, I do vaguely recall slashing up corpses with a knife in the original Soldier of Fortune when I was a kid.
Usually I'm pretty mellow in games as well (the Fallout 3 thing is sorta an anomoly for me). I actually felt horribly guilty after pillaging a town in Mount and Blade when I needed money/supplies. And I can NEVER play an evil character if given the choice, because I can't bring myself to choose the evil choices :P.
My roommate, however, does some unintentionally horrible things in the name of justice, especially in Mount and Blade. For instance, during battles if he saw someone who looked ragged or poor, he would decide that that person needed sparing. Unfortunetly, the only way of ensuring they didn't die was to knock them out. And the only practical way of knocking them out...was running them over repeatedly with his horse. Which he did. Many many times. As his entire army looked on.
Then he made a point of hunting up a ransom broker instead of a slave trader, so that the man could go back to his family.
Oh, and he has a famous story about playing Star Wrs Bounty Hunter. Someone ticked him off, so he tazed them, tied them up, set them on fire, then threw them off a cliff, in that order. And don't even get me started about his stories about Mercenaries (one involves him rifle-butting an old lady to death trying and get her to move off a pile of explosives he set)
I don't know whether to admire him or be afraid of him :P
