Cosmetic items, from what I understand. The only P2W items are stash tabs, but even so those are only P2W in the loosest sense i.e., there are alternatives, albeit more troublesome ones, to storing items besides buying more stash tabs. Otherwise, some of the things that you can purchase with in-game credits, as confirmed by the devs, include clothes dyes, extra gore, pets and sex changes.
From my experience, most people don't have pets. For every 20 people I see in-game, maybe 2 have a bronze or normal kiwi, and 1 has a silver kiwi. Gold and Diamond ones are rare. Kiwis are also not available after the pre-order period, so that's an extra incentive to get it now if you want to be outstanding when the game hits OB.
PoE's maps are very linear and straightforward. You will never get lost, though some are so expansive that it will take you a while to find the exit. Otherwise the maps are very linearly connected. My topography skills are shit, and I never found myself getting lost in-game or wandering about aimlessly :P
There are only 2 problems with the game right now: 1) rubber banding mobs is quite a prevalent issue on my end, but the devs are already on it I believe; and 2) the initial part of the game is quite slow, but they are also working on improving this. It gets really fast once you hit level 20-30, because you will be hoarding Cast Speed other active skill gems to improve your DPS. My Spark skill went from 1 attack per 0.7+second to 1 attack per 0.43 second lol.
