If I recall correctly, Seasons 3 and 4 are actually Book 3 divided roughly in half. Also some of book 5 (I think - isn't that when Reek came in? They producers didn't want to have a character "disappear" so they worked that into the timeline).

They removed the Bastard of Bolton from Season 2 and added him back in there when it covers book 5 stuff or if I remember right most of it wasn't in the books but brought up in conversation and Theon's/Reek's thoughts in Dance
